Today's risc-v psABI meeting brought up the topic of CFI-related ABI
changes, and FreeBSD wasn't worried about jmp_buf because they already
had twice the space we do (OpenBSD matches us, presumably because they
too just picked "the next power of two" over what they actually
required), and glibc wasn't worried because they have a hilariously
large sigset_t that they can steal space from (and apparently already
did to support the x86-64 CET shadow stack).
So rather than continue to assume that our minimal amount of free space
will suffice, let's just double it while our ABI isn't yet fixed, and
know we won't have to worry about it in a couple of years time when we
actually have riscv64 hardware shadow stack.

If we switch from x18 to gp for shadow call stack, we're going to need
another slot in jmp_buf. We'll need this even for hardware shadow call
stacks too.
While I'm here, and because this is likely my last chance, let's just
round this up to 32 for safety. musl and glibc only have the minimum
needed (which I think means they'll need an ABI break to support SCS
unless they just use a callee-saved general purpose register), but since
we can't do ABI breaks after we ship, let's play it safe.

setjmp() and _setjmp() were already correct, but we'd missed
sigsetjmp(). Found while investigating a toybox timeout(1) failure on
macOS, which doesn't annotate _any_ of these functions.
Also add all the missing doc comments while I'm here.

The NDK only supports >= 16, so remove anything older than that to avoid
giving the misleading impression that such old targets are still
supported.
(This change doesn't touch <unistd.h>. I'll follow up with that once the
outstanding FORTIFY changes to that file are in.)

When building the bionic tests, we use -fno-builtins, which turns off
clang's recognition of vfork as a magical function that can return
twice. Explicitly annotate our returns_twice functions to ensure that
we generate correct code even in -fno-builtins (and while we're at it,
annotate the no_return functions as well, so we generate faster code).

POSIX says it's unspecified whether setjmp is a macro or a function,
but C11 says it's a macro, and the C standard always wins.

<machine/asm.h> was internal use only.
<machine/fenv.h> is quite large, but can live in <bits/...>.
<machine/regdef.h> is trivially replaced by saying $x instead of x in
our assembler.
<machine/setjmp.h> is trivially inlined into <setjmp.h>.
<sgidefs.h> is unused.

This shouldn't be public API, isn't supported on x86/x86_64, and it's
unlikely anyone would have actually seen the message before anyway.
Using __libc_fatal makes it much more likely to be seen.
