This was initially just to try on "pty" and "tty" for size, while
discussing with other projects to try to align on vocabulary, but -- in
the implemention and tests at least -- but these work out so well that
I'm tempted to go with them anyway if we can't come to a concensus. We
can always come back and change them again later.
What I really like is that you pass the pty to functions like ptsname()
and grantpt() with a 'p' in the name, and the tty to functions like
ttyname() and tcsetattr() with just 't's.
The use of "parent" and "child" in forkpty() seems helpful too.
Also fix the documentation of forkpty(), which wasn't quite right.

I'm skeptical about the usefulness of this, but it's in POSIX, it's
in glibc (but not iOS), and it is used in some internal source (test
runners and container code).

Thread local buffers were using pthread_setspecific for storage with
lazy initialization. pthread_setspecific shares TLS slots between the
linker and libc.so, so thread local buffers being initialized in a
different order between libc.so and the linker meant that bad things
would happen (manifesting as snprintf not working because the
locale was mangled)

glibc, FreeBSD, OpenBSD, and Darwin all just leave the fd unchanged and
possibly uninitialized. Setting it to -1 seems friendlier, though.

Based on the package/apps/Terminal implementation. I'll switch them over

Also clean up the implementation of all the pty functions, add tests,
and fix the stub implementations of ttyname(3) and ttyname_r(3).
