Now the NDK doesn't support API levels below 21, we don't actually need
the different arm32 vs x86 annotations. In general we haven't been
removing this historical information because it might be interesting to
someone, and there's no real reason to remove it, but we've had
versioner bugs recently with these more complex cases.

The next NDK to take these headers only supports API 21 and later, so
clean up some of the trivial cruft.

I had hoped that this would then let us remove more of the "introduced
in" annotations, but it looks like that's not really going to happen
until the NDK's minimum supported API is 21.
Also remove a .c file that wasn't referenced anywhere.

Historically we've made a few mistakes where they haven't matched the
right number. And most non-Googlers are much more familiar with the
numbers, so it seems to make sense to rely more on them. Especially in
header files, which we actually expect real people to have to read from
time to time.

The NDK only supports >= 16, so remove anything older than that to avoid
giving the misleading impression that such old targets are still
supported.
(This change doesn't touch <unistd.h>. I'll follow up with that once the
outstanding FORTIFY changes to that file are in.)

<machine/asm.h> was internal use only.
<machine/fenv.h> is quite large, but can live in <bits/...>.
<machine/regdef.h> is trivially replaced by saying $x instead of x in
our assembler.
<machine/setjmp.h> is trivially inlined into <setjmp.h>.
<sgidefs.h> is unused.
