To enable experiments with non-4KiB page sizes, introduce
an inline page_size() function that will either return the runtime
page size (if PAGE_SIZE is not 4096) or a constant 4096 (elsewhere).
This should ensure that there are no changes to the generated code on
unaffected platforms.

Plain __ for generated syscalls didn't mean it was a hidden symbol, it
just meant "please don't use this". We added ___ to signify that a
hidden symbol should be generated, but then we added the map files
anyway so you now have to explicitly export symbols. Given that, this
convention serves no particular purpose so we may as well just use the
nicer names have everything look the same.

While this was never an inline, this function alone has caused most of
the bug reports related to _FILE_OFFSET_BITS=64. Providing an inline
for it should allow a lot more code to build with _FILE_OFFSET_BITS=64
when targeting pre-L.

This removes another way to obtain objects larger than PTRDIFF_MAX. The
only known remaining hole is now jemalloc's merging of virtual memory
spans.
Technically this could be wrapped in an __LP64__ ifndef since it can't
occur on 64-bit due to the 1:1 split. It doesn't really matter either
way.

The mremap definition was incorrect (unsigned long instead of int) and
it was missing the optional new_address parameter.
