Should use android/log.h instead of cutils/log.h as a good example
to all others. Adjust header order to comply with Android Coding
standards.

When formatting a blank SD card, it needs to set MBR signature when creating MBR.
If not, the 'format' operation will fail.

The values need to be converted to loff_t *before* performing
the multiplication, else any sector offset past MAX_INT
bytes will overflow.
